Here is
the email version of the brochure and registration form for DAIS's 11th Annual Multicultural Student Leadership Conference,
which is Saturday, May 15, 2010, at the Hackley School. This form is to be used only by students who attend schools
that have not received a faxed or emailed version. Check with your advisor to see if they have received faxed or emailed information
about the conference. Students who attend schools that will be taking care of registration and payment should not email
this form directly to DAIS! Follow your advisor's directions for registration.
If you are
registering for the conference individually through email and not as part of a group from your school, please make sure
to provide all of the required information and to send the form and the registration fee to the address listed for DAIS.
