and registration materials for DAIS's 12th Annual Multicultural Student Leadership Conference, on Saturday,
May 14, 2011, at the Riverdale Country School, can be downloaded by clicking the links below.
The group
registration download is available to be used by schools registering a group of students for the conference.
The student registration download is to be used only by students who attend schools that have not received a faxed
or emailed version. Check with your advisor to see if they have received faxed or emailed information about the conference.
Students who attend schools that will be taking care of registration and payment should not email or send this form
directly to DAIS! Follow your advisor's directions for registration.
If you are
registering for the conference individually through email or the downloaded form and not as part of a group from your
school, please make sure to provide all of the required information and to send the form and the registration fee to the address
listed for DAIS.
Early registration is through April 13, 2011.
Regular registration
is after April 13, until the conference on May 14, 2011.
